2. Backup to more than one destination.(i.e.. Tape & Hard Drive or DVDR/W)
3. Have a media rotation plan and stick to it.
4. The same person or persons should be responsible for Daily Rotation of media.
5. Store a Copy of your Backup off site.
6. Replace Tape media every year.

You need a formal backup plan. Backups should be automated and verified on a regular basis, so that you're not caught by surprise when a catastrophic failure occurs.
